Complete a Comprehensive Mental Health Screening

Your mental well-being matters. This short, confidential questionnaire helps identify signs of common mental health concerns. It’s not a diagnosis, but it can be a step in understanding how you’re feeling and deciding if additional support could help. Honest answers ensure you receive the best resources and guidance possible.

The screening begins by asking questions about who you are and whether you have been treated for various mental health conditions. Then it asks about the following topics:

  • Anxiety and Depression
  • Suicidal thoughts
  • Trauma and PTSD
  • Substance Use
  • Eating disorders

After the screening, you'll be offered a listing of resources that could help - including supportive options right here on campus and in our local community.

Please note: If your results indicate you may be experiencing symptoms of a mental illness, consider sharing your results with someone. For Carolina students, an initial assessment at CAPS offers increased understanding of your full needs and an opportunity to discuss options for how to feel better with a professional.
